Self Referral

Most referrals to other specialists will require you seeing or speaking with a GP. However, there are a number of local services, listed below, that you are able to make self referrals to.

Podiatry

Sutton Health & Care Podiatry Service supports patients with foot and lower limb problems to maintain and improve their mobility, and help them remain independent. We also treat and prevent complications for people considered at “high risk”.

Community Musculoskeletal Service

The getUBetter app is now available to all patients. It’s a free, easy-to-use tool helping you to self-manage your muscle, bone, and joint (MSK) injuries and conditions.​New joint and muscle problems are common and include shoulder, neck, back and knee problems. Many people will have had these at some point in their life. Most of these problems are not serious and improve or get better quickly with self management.

Falls Prevention Service

The Falls Prevention Service provides assessment, advice and exercise modalities for people aged 65 and over who are at risk of falling.The Falls Prevention Service aims to prevent falls and unnecessary admission to hospital by seeing a patient before an injurious fall occurs or by supporting patients with a maintenance programme after rehabilitation. We do this by providing assessment, advice and exercise modalities to increase a person's ability to move safely.

Healum Hive App

App to empower people with long term conditions to understand and manage all of their health choices. It supports people to manage their health and to develop healthy behaviours in partnership with the healthcare professionals that deliver their care.

The Sun Project

The SUN Service is a service for people who experience emotional and behavioural difficulties, sometimes referred to as personality disorder.The SUN Service aims to help people feel less isolated and overwhelmed by emotions or problems.It also aims to help people to learn better coping skills and reduce harmful behaviours such as self injury, drug and alcohol use.

Sutton Sexual Health Services

The Rosehill Clinic is the main clinic and provides sexual health, contraception and HIV services. We are located in the main St Helier Hospital building, on the first floor of B block. We are currently unable to provide walk in services but we do offer emergency and routine appointments.

getUBetter

The getUBetter app is provided free of charge by Sutton CCG for patients registered at Maldon Road Surgery. It can be accessed on a Smartphone or on the Web and guides you day-by-day through a sequence of exercises & tips to help you get better from a range of new or recurrent musculoskeletal conditions. You will be referred to the App by one of our GPs or Physio. P

Sutton Carers Centre

Anyone could be a Carer: an 80-year-old woman looking after her husband who has Alzheimer’s disease, a 40-year-old man caring for his partner who has terminal cancer, or a15-year-old girl looking after a parent with alcohol dependency as well as your little brother who has autism Our services aim to help ensure that no Carer has to reach crisis point before they get the support they need.

You can refer yourself directly to a psychological therapies service (IAPT) without seeing your GP.These services offer therapies, such as cognitive behavioural therapy (CBT), for common problems including: stress, anxiety, depression, obsessive compulsive disorder (OCD) and phobias
